  • For each owner and authorized signer: name, address, date of birth, Social Security number or Tax ID number, citizenship info
  • E-mail address and phone number
  • U.S. address and owner establishment date
  • Legal name of business and type of business

Additional documentation may be required depending on business structure. See Business Services Department for membership eligibility requirements.
